He is survived by: his children, Jennifer Slack (Stanley Clark), Edward Slack and Christal Da Mata (Rafael); his grandchildren, Colleen McIntyre (Joeseph), Samuel Miller (Payton), Brady Slack (Gianni), Torri Towles (Michell), Dale Miller (Haven), Jonathan Slack, Abrielle Guzman and Ridge Guzman; and his great grandchildren, Hailee Forster, Ava Notestine, Jamison McIntyre and Allison Miller.
