He is survived by: his wife Verla Cook; his children, DeEtt Andrews (John) of Quincy, Derek Cook of California, Paige Mullen (Tim) of Tennessee and Anjanelle Sikkenga (Todd) of Grand Rapids; his grandchildren, Nolan Andrews (Melissa), Tyler Andrews, Patricia Cook (Sean), McKennah Williams (Matt), Alec Mullen (Caroline), Logan Sikkenga (Paige) and Tiffany Schuitema (Travis); his great grandchildren, Gabriella, Cambria, Asher, Cameron and Violet; and his siblings, LeRoy Cook (Betty Clements), Margaret Wagner (Al) and Kim Cook (Sue).
