She is survived by: her daughters, Jen Adgate (Rich) of Blanchard and Carla Kolka (Josh); her grandchildren, Olivia Adgate, Sydney Adgate, Trinity Kolka and Dawson Kolka; her mother Marilyn Chalmers (Chalmers); and her brothers, Dale Herbek Jr. (Anita), Ken Herbek (Lorie) and Rick Herbek (Mary). She is also survived by many nieces and nephews.
