He was predeceased by: his parents, Marvin Cook and Rita Cook (Shuda). He is survived by: his wife Renee; his sons, Courtney Cook (Stephanie), Ryan Cook (Rachell) and Justin Cook (Jessica); his grandchildren, Alyssa and Austin (Kayla); his great grandchildren, Ryker, Tristin, Bryce, Avery, Hannah, Ace, Olivia, Khloe and Charleigh; and his close friend Roman Kujinski. He is also survived by three sons, ten grandchildren, and one great-grandson.
