He was predeceased by: his parents, Samuel Gill Phillips II and Juanita Emogene Phillips (Ledbetter); his siblings, Karen Krakowiak, John Phillips, Doug Chance, Terry Rey and Rohn Elsner; and his niece Amy Krakowiak.
He is survived by: his wife Bonnie; his sons, Kevin and Steven; his daughter-in-law Jacqueline; his grandchildren, Stephen, Kendall, Karley and Kelvin; and his siblings, Joe (Vinzanna Phillips), Lynn Chance, Ruth (Dave Dillie), Phil Phillips (Mena), William (Holly Rey), Betty Rey, Linda Elsner and Jan Krakowiak. He is also survived by many nieces, nephews, great nieces, great nephews, cousins, and friends.
