She is survived by: her husband Robert Slezak (Bob); her children, Catherine Bender (Bryan), David Slezak (Christine), Patricia Shiflett (Thomas), Kenneth Slezak (Stephanie) and James Slezak (Kathleen); her grandchildren, Cameron Shiflett (Jaynie), Brian Slezak (Meghan), Matthew Slezak (Jessica), Ashley Moyer (Michael), Blake Shiflett (Alexandrea), Emily Mosier (Trevor), Michael Bender, Jacob Slezak (Megan Stone), Bridgette Slezak, Zachary Slezak, Christian Bender, Gabrielle Slezak, Max Slezak and Jack Slezak; her great grandchildren, Noah, Emma, Lily, Beckett, Miles, Luka and Eli; her sister Carol Scovotti (Ross); her niece Rhonda Smith (Robert); her nephews, Tom Slezak Jr., Steven Slezak (Jaime), Greg Hess (Tammy) and John Hess (Jackie); and her friend Dorothy Miklosovic of Brunswick, Ohio.
