She was predeceased by: her parents, Walter Kollar and Ruth Kollar (Shupert); her husband Grover K. Armentrout; and her siblings, John Kollar, Lou Kollar, Mike Kollar and Tom Kollar.
She is survived by: her children, Larry Armentrout (Kathleen), Tony Armentrout and Mitch Armentrout (Vivian); and her siblings, Bill Kollar and Gale Kollar. She is also survived by seven grandchildren; 10 great-grandchildren.
Memorial donations may be made in Phyllis's name to Hospice of Holland, 270 Hoover Blvd., Holland, MI 49423.
