He was predeceased by: his spouse Paraskewia Kuzmiw; his son Roman Kuzmiw (Linda); his sibling Zenon; and his siblings-in-law, Wasyl Korz and Anna.
He is survived by: his daughter Anna Collins (Richard); his grandchildren, Steven Collins (Lisa), Michael Collins, Andrea Kuzmiw, Emily Kuzmiw (Mark Wingo, Jr.) and Danielle Kuzmiw; his great grandchildren, Haley Tolley, Nikolai, Alek Wingo and Jacob Collins; his siblings, Wolodymyr, Roman, Maria and Sedonia; and his siblings-in-law, Olha, Michael Tymiuk, John and Taisa Korz. He is also survived by numerous nieces, nephews, cousins & friends.
