He was predeceased by: his parents, James G., Sr. and Shirley A. Zeltman Formwalt. He is survived by: his siblings, Karen L. Roach (Jim), Kim A. Bair (George) and James G. Formwalt, Jr. (Jay); his nieces, Heather Bateman, Kristen Carter, Lauren Mason (Kevin) and Kaitlin Deal (Jon); his nephew Joshua Formwalt; and his great-nieces and great-nephews, Sulli, Alli, Lizzie Bateman, Aaliyah, Paige Mason, Eliza, Maris Carter, Brantley, Cayson Deal and Finley Carter.
