He was predeceased by: his parents, Anthony F. "Cappy" Jakubiak of Worcester and Genevieve Elizabeth Jakubiak (Krawczyk) of Webster; his brother Ronald S. Jakubiak of Worcester; his son Ronald A. Jakubiak of Millbury; and his former wife Rita M. Jakubiak (LeBlanc) of Millbury.
He is survived by: his children, Marie G. Jakubiak-Shorette of North Grafton, Jessica L. Jakubiak, Michael J. Jakubiak, Jeffrey J. Jakubiak (Laura Lohnes) of Millbury and Johnathan Westfall of Worcester; his grandchildren, Matthew, Mallory, Anthony, Jazzy, Tiana, Mark and Caitlynn; his great grandchildren, Lukis, Timothy Jr. and Waylon; his nephew Bobby Jakubiak of Arizona; and his friends, Richard Chysna of Worcester, Pat, Charlie of Rhode Island and John Shorette Jr. of Millbury. He is also survived by nephews and nieces.
