He was predeceased by: his parents, Prescott D'Onofrio and Ellen D'Onofrio (Blue); his brothers, Ronald D'Onofrio and John D'Onofrio; his former wife Ginetta D'Onofrio (D'Angelo); his wife Virginia; and his stepson Daniel K. Mitchell (Patricia).

He is survived by: his children, Richard P. D'Onofrio Jr. (Joanne) of Norton and Jean M. Fitzpatrick (late Joseph J. Fitzpatrick) of Norton; his grandchildren, Brittany L. Precourt (Nick), Thomas W. Durden III (Jenn), Brooke E. Durden, Katie M. D'Onofrio and Richie P D'Onofrio; his great grandchildren, Kaia, Valentina, Ayla and Nora; and his stepdaughters, Linda M. Shackford (Josh) and Donna R. Leonardo (Vincent). He is also survived by many nieces and nephews.
