He was predeceased by: his mother Joan Pavlak (Peterson). He is survived by: his father Edmund Pavlak; his wife Jeannine Pavlak (Boisvert); his children, Thomas Francis Burns IV (Anneliese), Allyson Marie Burns, Caitlin Nichole Pavlak, Ethan Robert Pavlak and Anna Leigh Pavlak; his siblings, Kathy Kraner, Susan Borsella of Chester, MA, Joe Pavlak (Linda) of Eastport, NY and Aimee Lunden (Eric) of Agawam, MA; and also Ty Davis of Las Vegas, NV. He is also survived by other loving relatives and friends.
In lieu of flowers, his family requests donations be made in Eric's name to Mission 22, 694 N. Larch St. #910, Sisters, OR 97759.
