He was predeceased by: his brother William Fivek. He is survived by: his wife Barbara Gwinner Fivek; his sons, David Milton Fivek of Concord and Jason Allan Fivek (Rory) of Concord; and his grandchildren, Kyle, Ryan, Alex and Bridget.
