He was predeceased by: his parents, Peter Maxwell Phillips and Rosa Lee Thomas Phillips. He is survived by: his children, Allison Michelle Phillips Peters (Brandon) and Chase Michael Phillips (Rebecca); his grandchildren, Madison Elizabeth Phillips, Reily Maxwell Phillips, Allie Michelle Peters, Ella Mary Marie Peters and Colin Henry Phillips; and his sisters, Krista Kay Phillips Connell and Carla Jeanne Phillips Lee.

In lieu of flowers, memorial donations may be made to the Huntington's Disease Society of America, St. Jude Children's Research Hospital, or the charity of one's choice.
