He was predeceased by: his father O. C. Perkins. He is survived by: his wife Hope Perkins; his children, Tori, Kori, Jessica, Sierra and Trystanlee; his friend Chad Parker; his mother Betty; and his brothers, Aaron, Adam and Allen.
