He was predeceased by: his parents, Roy Cook and Shirley Cook; his sons, Dalton S Cook (Jarrett Cook) and Brother Roy Lee Cook; and his niece Franchesca Cook.

He is survived by: his daughters, Sehran Broussard, Jalisa Cook, Jasmine Cook and Jessica Washington; and his siblings, Roslyn Cook, Kathleen Cook, Leroy Cook Jr., Carlton Cook and Wilton Cook (Stephanie).
