He was predeceased by: his wife Alice Mallard; and his parents, Courtland Mallard and Boyce Mallard. He is survived by: his son Jay; his daughter-in-law Fran Mallard; his daughter Sherri Jurak; his grandchildren, Jarred Mallard (Allie), Ashley and Jake Jurak; and his great grandchildren, Branson, Case and Reagan. He is also survived by several nieces and nephews.
In lieu of flowers, the family request memorials be made to the Antioch Methodist Church in West Monroe, LA in care of Mrs. Rogers at 1030 Okaloosa Rd. Eros, LA 71238.
