He was predeceased by: his parents, Jack Lewis and Sandra Lewis; and his brother Gene Lewis. He is survived by: his wife Judy Lewis; his brother Jim Lewis; his grandchildren, Sam Cates, Ben Cates and Addelind Cates; his stepchildren, Juanita Cates and Damon Cates; and his uncles, Terry Lewis and Rick Lewis. He is also survived by extended family and friends.
