It is with great sadness that we announce the death of Jakob Ray Duchnowski (Salyersville, Kentucky), who passed away on May 6, 2021, leaving to mourn family and friends. Family and friends are welcome to send flowers or leave their condolences on this memorial page and share them with the family.

He is survived by: his parents, Joshua Sr. Duchnowski and Karissa Duchnowski; his siblings, Connor Duchnowski, Joshua Duchnowski Jr., Adaleiha Duchnowski and Makenna Duchnowski; and his grandparents, Robert Duchnowski Sr., JoMarie Duchnowski, Dale Mason and Tracey Cline. He is also survived by several aunts, uncles and cousins.

COVID 19 restrictions are enforced for public safety.
