He is survived by: his wife Janice Sizemore Collins; his daughter Renee Collins Smith (Tim) of West Liberty, Kentucky; his stepdaughter Wendy Hacker of Hazard, Kentucky; his grandson Kordell Smith of West Liberty, Kentucky; his stepgranddaughters, Lucy and Rachel Duff of Hazard, Kentucky; and his pet Dexter. He is also survived by extended family and friends.
