He is survived by: his wife Charity Burgess; his daughter Keri Hamilton (Michael) of Mayfield, KY; his father Jerry Burgess of Mayfield, KY; his siblings, Keith Burgess (Sabrina) of Mayfield, KY and Christa Lee (Barry); and his nieces, Brittany Hoback and Brianna Burgess.
