With heavy hearts, we announce the death of Carol Cook Johnson (Mayfield, Kentucky), who passed away on May 11, 2023 at the age of 83. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her former husband John C. Waters, Jr.; her parents, LG and Dorothy Cook; and her brother-in-law Larry McIntosh. She is survived by: her husband Don Johnson; her daughters, Allyson Purdom (Tripp) and Lynne Caulkins (Gregory); her siblings, Mary Ann McIntosh and Greg Cook (Tammy); her grandchildren, Paige Purdom Giles (Ronnie), Wells Purdom, Scott York and Reed York; and her great grandchildren, Maddox Giles and Cook Giles.
