He is survived by: his children, James A. Hazelwood of Leitchfield, Jason Hazelwood of Leitchfield, Tasha Hazelwood of Leitchfield and Matthew Hazelwood (Amy) of Missouri; his brother Mark Hazelwood (Renee); his grandchildren, William, Alandreia, Tyler, Steve, Hayden and Damian; his great grandchildren, Naudalynn and Jackson; his niece Cassandra "Jennie" Johnson; and his brother-in-law Ted Renfrow.
Expressions of sympathy may take the form of donations to the Dermitt Funeral Home to assist with final expenses.
