He is survived by: his wife Darlene Elkins Cook; his stepsons, Kenneth Sams of Lawrenceburg and Scott Sams (Stephanie) of Frankfort; and his siblings, Rickey Wayne Cook (Sharon Kay) of Louisville and Jerelina Mae "Jerri" Stumph (Carl) of Willisburg. He is also survived by 4 step grandchildren, several nieces, nephews, great nieces and great nephews.
