He was predeceased by: his parents, Jessie Loretta Lewis and Harold Middleton; his stepmother Ruby Middleton; his grandmother Alice Stewart Middleton; his brothers, Bill, Terry and Jerry Middleton; and his son-in-law Randy Middleton.
He is survived by: his wife Carolyn Winders Middleton; his daughters, Dana Middleton and Allison Middleton Harrison (Bo); his grandsons, Robbie Middleton and Jake Middleton (Victoria); his great grandchildren, Isabella, Tucker and Lucy Middleton; his siblings, Wilma Middleton, Dorothy Middleton Napier, Stella Middleton Grills, Yvonne Middleton Owens, Vickie Middleton Owens (Charles) and Larry Middleton; and his close friends, Danny, Lira McCreary, Deacon, Herbert Dean, Bo Harrison, Chad Brock and Steve Duff. He is also survived by nieces and nephews, other family and friends.
