He is survived by: his wife Debra Harvey Perkins of Flatwoods; his children, Kevin Ray Perkins (Amber) of Cynthiana, Curtis Allen Perkins (Amy) of Georgetown, Daniel Perkins (Kristy) of Cynthiana, Elijah Ehrlich of Flatwoods and Mary E. Barker (Joshua) of Flatwoods; and his siblings, Donald Perkins of Cynthiana, Jeff Perkins of Cynthiana and Cindy Perkins of Cynthiana.
