He is survived by: his children, Nikki Cook of Elizabethtown, Eric Cook of Elizabethtown, Jason Cook (Charlene) of Roundhill and Daniel Cook (Andrea) of Radcliff; and his siblings, Brenda Hornback of Elizabethtown, Mona Wolfe of Upton, Teresa Hawkins of Upton, Hayden Cook of Sonora, Dudor Jenkins of Upton and Glennie Jenkins of Upton. He is also survived by eight grandchildren.
