She is survived by: her children, Hunter Ray and Isabelle Rose Sweazy of Stephensport, KY; her mother Lisa Childress of Elizabethtown, KY; her sisters, Angela Self of Louisville, KY, Sara Ashley (Johnathan) of Stephensport, KY and Megan Childress of Elizabethtown, KY; her boyfriend Ivan Ray Sweazy, Jr.; her grandmother Doris Childress; and her nieces and nephews, Brandon Self, Ella Weick, Jase Ashley, Elijah Childress and Alinna Gonterman.
