He was predeceased by: his parents, Henry Harrison Cook and Laura "Demi" Allen Cook. He is survived by: his wife Sylvia Sue Hensley Cook of Mount Vernon, KY; his children, Angelia Cook of Berea, KY, Jamie Alcorn (Joseph) of Mount Vernon and Jody Hayes (Melvin) of Mount Vernon, KY; his grandchildren, Lyndsey Cook, Madison Hayes, Cole Powell and Esme Alcorn; and his siblings, Stanley Cook (Tonya) of Livingston, KY, Dale Cook (Tobie) of Waco, KY and Edna Turner of Lebanon, VA. He is also survived by nieces, nephews, friends.
