He was predeceased by: his parents, John Cook and Dorothy Cook (Mitts); and his brother Leonard Cook. He is survived by: his children, Shannon Roseberry (John) and James Shawn Cook; his grandchildren, Mason, Holden, Dalton Roseberry, Michael and Kelsey Cook; and his siblings, John Cook, Jr., Larry Cook, Charles Cook, Carol Mignot, Donna Locke, Connie Ward and Vicky Carson.
