He was predeceased by: his parents, Cyril Thomas and Theresa Ann Forge (Burick); his brother Cyril T. Forge II; and his brother-in-law Bruce Hollister. He is survived by: his sisters, Joan M. Hollister of Leavenworth, Janet M. Forge (Michael) of Fairway and Julie A. Burkhart (Philip) of Shawnee, Kan.; his pet Coco; and also, Becky, Joe and Charlie.
