She is survived by: her parents, Larry Michael Jack and Deborah June Smith (Gary); her siblings, David Jack (Lisa), Jason Jones (Ilia), Christina Little (Nathan), Chris Cheng (Phuong) and Jason Cheng (Jennifer); and her caregiver Sonia Amezcua. She is also survived by other uncles, aunts, cousins, nephews, and nieces.
