He was predeceased by: his parents, Henry Cook and Alice Cook (Esson). He is survived by: his wife Peggy Ballinger; and his children, Mike Cook, Daniel Cook, John Ballinger and Jesicca Leapaldt. He is also survived by grandchildren.
