He was predeceased by: his parents, Herman W. Koontz and Myra S. Koontz; and his brother Kenneth D. Koontz. He is survived by: his wife Alice E. Koontz; his sons, David (Linda) of Winona Lake, Doug of Warsaw and Steve (Christine) of Winona Lake, Indiana.; and his grandchildren, Ashley Miller (John), Beth, Kate, Scott and Michael Koontz. He is also survived by children and becoming grandparents and great grandparents.
