He is survived by: his wife Patsy Hazelwood (Feltner); his children, Renee' Sparkman (Josh) of Scottsburg, Eric Hazelwood of Scottsburg and Jennifer Roll (Ethan) of Scottsburg, Indiana; his grandchildren, Alex, Brandon, Kaden, Grant, Erionna, Sadie, Sydney, Sedric, Simon, Cameron and Greyson; and his sisters, Nancy White of Scottsburg, Eva Johnson of Scottsburg, Martha Alvey of Seymour and Ella Sturdevant of Austin, Indiana. He is also survived by several nieces, nephew and cousins.
