He was predeceased by: his father Robert D. McIntosh; and his brother David McIntosh (Dona McIntosh). He is survived by: his mother Sandra Cook (Schneider) of Westfield, Indiana; his children, Roman McIntosh (Amber) of Louisville, Jordan McIntosh of Louisville, Kentucky and Amanda McIntosh of Louisville, Kentucky; his grandchildren, Colt, Addy and Ida; his significant other Gina Harper of Scottsburg, Indiana; and his nephew Joe McIntosh of Owensboro, Kentucky. He is also survived by several nieces, nephews, cousins and many friends.
