He was predeceased by: his father Thomas Middleton; his son Donald Middleton; his sister Judy Middleton; and his siblings-in-law, Brian Casteel and Gayle Burris.
He is survived by: his mother Nancy Lewis Middleton; his wife Claudia G. Burris Middleton; his sons, Richard Mann, Jason Rettinger, Joshua Rettinger and Justin Rettinger; his grandchildren, Cheyenne Middleton, Thomas Middleton, Marissa Middleton, Tessa Rettinger and Jason "Bighead" Rettinger; his siblings, Kevin Middleton, Sandy Peters, Paula Casteel, Carl Lewis and Wendy Baker; and his close friends, Jacob Mello and Zay'Lynn Jones. He is also survived by 4 great grandchildren, nieces; nephews; cousins; and many friends.
