He was predeceased by: his parents, Howard D. Stevens and Ferrol Jean Stevens (Polm); his parents-in-law, John L. Moon and Norma J. Moon; his siblings, Rex Howard Stevens, Robert J. Stevens (Mary Lou) and Susan Scheidt (late Tom); his half-sister Berthala of Maine; his niece Genny Stevens; and his nephew Phillip King.

He is survived by: his wife Carolyn S. Moon; and his son John Stevens (Misty Ortiz) of Fortville. He is also survived by several nieces, nephews, and cousins.

Memorial contributions in Ted's name may be made to the Fairmount Masonic Lodge #635, 112 1/2 N. Main St., Fairmount, IN 46928.
