She was predeceased by: her parents, Charles B. Hook and Margaret H. Hook (Zarse); her husband Howard Swafford; and her brothers, Phil and Greg Hook.
She is survived by: her children, Kenneth Swafford (Lori) of Kewanna, Diana Miner (Franklin) of Logansport and Karen McCollough (Loren) of Lakeville; her sisters, Dorothy DeWitt of Logansport and Gwen Gore of Logansport; her grandchildren, Kevin, Brandi (Dan), Nick (Ashley), Howard, Ian, Fayette and Shelby; and her great grandchildren, Jaden, Jacob, Madison, Aubrey, Jayson, Elijah, Kayden, Aleah and Natalynn.
Memorial contributions may be made to Riley's Children's Hospital and/or the Kewanna VFW.
