He was predeceased by: his parents, Harold Cook and Anita Cook (Fitzgerald); and his siblings, Joe Cook and Darlene Loveless. He is survived by: his wife Vivian Veach; his daughters, Terri Lynn Justice (Greg) of Lafayette and Marie Ann Howard (Steve) of Lafayette; his grandchildren, Nichole Thieke (Brandon), Heather Collins (Chris), Daniel Hunley (Deana), Taylor Justice (Rachel) and Shyla Hale (Josh); and his great grandchildren, Xzavier, Victoria, Zander, Chandler, Micah, Josiah, Hannah, Sarah, Elijah, Norah and Savannah.
