He is survived by: his mother Marjorie Irene Allen Bales (Williams); his stepfather Larry Bales; his sister Tanya Eubanks (Jerry); his stepsiblings, Deanna Thompson (Richard), Rick Bales (Elaine) and Steve Bales; his uncle Thomas "Tommy" Williams (Jill Quackenbush); and his aunts, Dottie Salsbery (Larry Weeden) and Jackie Pratt.
In lieu of flowers, memorial contributions may be made in Curtis' memory to the Church of God of the Union Assembly.
