He is survived by: his children, Daniel Mark "Opie" Hornyak, Jr. of French Lick, Tammy Hanes (Jeff) of Tell City, Celeste Hornyak of Kansas and Aimee Manning of Alabama; his brothers, Mark Hornyak of Whiting, Mike Svorcina of Chicago and Billy Hornyak of Hammond; and his grandchildren, Anthony, Michael, Danielle, Lyanna and Logan.
In lieu of flowers the family requests memorial contributions be made to Our Lady of the Springs Catholic Church, 8796 W. State Road 56, French Lick, IN 47432.
