He was predeceased by: his parents, Carrol Samuel Hazelbaker and Elizabeth "Bess" Hazelbaker (Shewmaker); his brother George Hazelbaker; his daughter-in-law Patricia Hazelbaker; and his grandson Kristopher Hazelbaker.
He is survived by: his wife Sandy Hazelbaker of Frankton; his children, Tami Pattengale (Glen) of Alexandria, Traci Pattengale (Glenn) of Parker City, Timothy Hazelbaker of Westfield and Tara Hazelbaker of New Orleans, LA.; and his brother Charles Hazelbaker (Nancy) of Markleville. He is also survived by granddaughter and great-granddaughter that they raised, Julie Gray, and Madison of Frankton; several grandchildren; numerous great-grandchildren and a great-great-grandson.
