He was predeceased by: his parents, Jesse H. Cook and Dorotha Cook (Riggle); and his siblings, Robert Cook, Daniel Cook, Joseph Cook and Vera Warfield.
He is survived by: his wife Devena Cook of Frankfort; his son Kenneth Cook (Susan) of Noblesville; and his siblings, Jesse Cook of Lafayette, John Cook of Kentucky and Kathleen Norris (Ed) of Bullhead City, AZ. He is also survived by several nieces and nephews.
