He is survived by: his mother Sonja Scotten (Donnals); his wife Laura Appleby (Crowe); his children, Tony Appleby (Sarah) of Hazelwood, Chris Appleby (Ronee) of Hazelwood and Amanda Summers (Patrick) of Paris, IL; his grandchildren, Christa Metheringham (Seth), Evan, Damien, Dominic, Isabelle, Eric, Adler, Egan Appleby and Finley Summers; his stepparents, Richard Scotten and Sue Appleby; his sister Theresa Kraus; and his half-sisters, Angie Appleby, Kearie Sheahan, Heather Honeycutt, Hollie Lone and Richelle Scotten-Bennett.
In lieu or in addition to flowers, memorial contributions may be made in David's name to Riley Children's Hospital.
