He was predeceased by: his mother Becky L. Polen. He is survived by: his father Bobby A. Polen of Indianapolis; his children, Isaac L., Ethan M. and Aubrey J. Corey; his significant other Jennifer; his brother Blake (Emily) of Brownsburg; his nephews, Austin and Logan; and his niece Brooklyn.
