He was predeceased by: his parents, Harry Smith and Amy Smith (Collins). He is survived by: his daughter Mandy Smith (Jason Floyd); his fiancé Christie Holstein of Deputy, Indiana; his stepdaughter Brooklynn Nickole Holstein of Deputy, Indiana; his brothers, Mike Smith of Georgia and Joseph Smith of Texas; and his grandchildren, Destiny Michelle and Chance Xavier Floyd.
