He was predeceased by: his parents, Tadeusz "Ted" Urban and Stanislawa "Stella" Urban; and his grandparents, Piotr, Michalina Urban, Stefan and Agnieska Lenart.
He is survived by: his siblings, Ed Urban (Diane), William Ted Urban and Christina Curry (Mike); his niece Christine Curry; and his great-nephews, Jayden and Camden. He is also survived by many aunts, uncles, and cousins.
In lieu of flowers, memorials in John's name can be made to the Sacred Heart Catholic Church.
