He was predeceased by: his parents, Albert Cook and Verna Cook; and his siblings, Albert Duane Cook and Marcia Papatyi. He is survived by: his wife Dian S. Wickwire; and his children, Eric Cook (Hallie) of Northlake, TX, Stephanie Cronin (Paul) of Beavercreek, OH and Jason Cook (Julie) of Bloomington, IL.. He is also survived by nine grandchildren.
In lieu of flowers, memorials may be given to the OSF Richard L. Owens Hospice Home.
