He was predeceased by: his parents, Walter "Pete" Mack and Agnes Mack (Schaechter); his brother Dennis Mack (Dolores); and his brother-in-law Tom Forsyth. He is survived by: his wife Janet K. Kelley; his children, Cynthia Mack, Pete Mack (Denise), Viola, Kelly House (Will) and Blue Grass; his grandchildren, Katelyn Mack (Bryan Liggis), Kyle Mack, Ryan Mack and Anthony House; and his sister Eleanor Forsyth of Oskaloosa. He is also survived by several nieces and nephews.
